Well written and really sheds a light on what really goes on. As a nurse, I truly realize just how important it is to set these patients up for a successful transition from the hospital to their home. We need to know who these patients are and understand the obstacles that are in the way for their recovery. Trying to engage with equality for all which seems to be a missing link. It is so important to make healthcare available to all. The navigation of our health care systems are very hard .It takes a team to help every person recover.
